Who can receive home health services?
Prescribed Home Health, Inc. is a Medicare licensed skilled provider. The patient must be home bound to received services. Home bound patients are physically or mentally unable or is a taxing effort to leave home (short of breathe, chair bound, unsafe to ambulate). Patients must be under the care of a physician and need assistance with Activities of Daily Living (ADL's), skilled nursing and/or therapy services.
This covers a wide range of patient suffering from a variety of illnesses. All home health service are administered in the comfort of your home. Check with our agency or your physician to see if you qualify for home health services.
Our job is to keep you out of the hospital and safe in your home and to teach you or your family member to manage your disease processes.
You will be kept informed concerning your condition.
We will help you obtain community services that you may quality for.
Your physician will be kept informed on any changes in your condition.
You will have access to a nurse 24-hours a day, 365 days of the year.
Medicare pays 100% of ALL home health services
